If you are interested in paying your monthly fees via credit card, click on the link below to complete an authorization form that we will keep on file which authorizes us to deduct your monthly payments.  For additional information regarding using this method of payment, please stop by the office.

If you have any difficulty opening the attached file (it is a MS Word file), please email jerseydevilsallstars@verizon.net and I will send you a different format to be able to print the form.
